Ageing is a common challenge in the European Union, which will increase the need for home care and healthcare services, thereby putting pressure on our welfare system. Understanding that ageing is a process that begins long before old age and, as postulated by the WHO, entails a progressive loss of intrinsic capacity influenced by lifestyles, enviromental factors, and chronic diseases. Preventing this disability is a goal of great interest within a more collaborative, integrated, and patient-citizen-centered vision that seems to offer a more sustainable and efficient response to current models. Innovative approaches to chronic pathology and prevention of functional decline will allow people to live longer and, above all, with better quality of life. Promoting participation and collaboration among all stakeholders in the care chain is essential
